You can submit your request through the handwritten Rebate Form. Print your Rebate Confirmation Page on finishing the form and mail it and a copy of your Invoice/Repair Order to GM Certified Service Rebates team whose address is printed on the Rebate Confirmation Page.

To know about current rebate offers, click “Current Rebate Offers” in the navigation bar of certified service page, and determine the rebate and Download Manual Form you need. After you complete the rebate form, you should mail the form, a copy of your Invoice/Repair Order to the address printed on the form.

Check the Status
After your submission, just wait for your prepaid card. Generally the delivery takes 6 to 8 weeks. You can check the status of your rebate through My Rebate Status. If you have any problem, call 1-877-476-8419 weekdays, 8am to 7pm CT.
